2010 Financial Commitment Form

We hope you'll keep in mind our congregational goal of stretching towards the Visionary or Fair Share Contribution of 5% or more of your adjusted annual income to ensure the success of this year's financial stewardship drive. We understand that 5% might not be possible for you. In that case, we hope you will able to consider being a Supporter, committing 2.5-2.9% your adjusted annual income, or a Sustainer committing 3.0-4.9% of your adjusted annual income.

Your adjusted annual income can be determined by taking your adjusted gross income, adding any tax-free income, and subtracting tax deductions, including non-reimbursed medical expenses, care of parent(s), costs of higher education, and cost of child care. (It is understood that at least some of these things will be estimates.)

Instructions: Please fill out the form below with the requested information about yourself, and your financial commitment to UUCL for 2010. When complete, press the SEND button to automatically record your financial commitment information as well as email it to the church office. After you press the SEND button, you will return to this page, with these large green type instructions replaced by an acknowledgement.

Fair Share Guidelines/Level of Financial Commitment

If one of the following describes your commitment, please select it.
   Supporter, (2.5%-2.9% of adjusted annual income).
   Sustainer, (3.0%-4.9% of adjusted annual income.)
   Visionary/Fair Share (5% or more of adjusted annual income.)

If you plan on paying your commitment monthly, you may find it convenient to have your payment automatically withdrawn from your checking account each month.
